Conditions
Upon purchasing a package we require your HR team to provide us with List of Employees Full Name and DOB and/or ID/Passport Copies who can avail the services.
Packages are valid for 1 year after date of payment; if usage is maxed within less than 1 year the company has the opportunity to purchase another package to avail the benefits, or employees will be offered PrimeCare’s regular pricing % discount applies ONLY to medicine, lab tests, and treatment prescribed after consultation.
10x, 20x, 30x indicate the number of employee registrations that the company can submit; however, if the same patient is sent multiple times, each visit will count toward the total number of consultation
